this is by a long shot.

I have being trying to locate this form my lecturer has told me its something to do with getting it signed by a doctor (and medical cert ??). Its going be attacted to a advise of absent form.

if anyone knows what im talking about much appreciated :confused:
i dont think i made any sense sorry.

A doctor can you give you a medical certificate.
But you need to visit the doctor while you are still sick.

I don't know what other forms you might need.

What are you studying?

You should get your medical certificate as soon as possible. Today!

The other forms come later. But you need to get a medical certificate while you are sick.

LOL nono i got my medical cert already
i submitted it with the advise of absent about a week ago

but the lecturer gave me call telling me i need fill in this other form and bring it back to the doctor to have it signed. Then re submited the form again

I had to have the doctor sign the form once and then submit it, but that was because I was unable to sit an end of semester exam. Seems strange for an assignment. I agree with pb, maybe ask the lecturer again, just to make sure.

its actually an exam :|. Yup thats exactly what i was talking about. The form was called "professtional authority form".
