well you know how you're about to orgasm, it's cause of the rhythmic build up. well say you weren't properly aroused, you still went for it, forced it, so the build up eventually comes, but when you reach the end and there isn't sufficient pressure on the penis head rim and the build up wasn't done properly, the orgasm is skipped over but the ejaculation arrives anyway. either it needs to be redone from the start or you continue in the hopes of orgasming which is much harder the second time around.
