These are very pedestrian questions.

1) plenty of info in any good textbook, outline the 3 main steps - initiation, propgation, termination

2) The chemical equation is:

C2H5OH (l) + 3O2 (g) --> 2CO2 (g) + 3H2O (l)/(g)

A peroxide is split to form two radicals. The radical undergoes an addition reaction with an ethene monomer to produce an activated monomer. This activated monomer reacts with another monomer to produce an activated dimer. This process continues until a desired chain length is reached. At termination, the activated ends of two chains react with eacher other to deactivate ( the free electrons form a bond), forming a polymer.
