I'm pretty sure you have to, though my teacher is doing all that for me anyways.

It doesn't have to be typed onto the major work itself, since it's written on the small coloured cards that we're attaching to each section of the project

The syllabus states on page 132:

'A cover sheet should be included which states the centre number and candiate number and the title of the work.'

So, yes.
