Trapezoidal Rule and Simpsons Rule (1 Viewer)

Okay so with trials quickly approaching, I just wanted to clarify a few things..

What are the formulae for the Trapezoidal Rule (one application and more than one application) and Simpsons Rule (one function value and more than three function values)..?

Trapezoidal: $\int_{a}^{b}f(x)dx=\frac{b-a}{2}(f(a)+f(b))$ : 2 function values (1 application)
and Simpsons (there are many but this is the easiest one and the one I use: $\int_{a}^{b}f(x)dx=\frac{b-a}{6}(f(a)+4f(\frac{a+b}{2})+f(b))$ : 3 function values (1 application)
