What do you want to do with your life?
Eng/med science is a maybe if you do biomedical engineering or something, I'm not really familiar with the area, but otherwise seems like a waste of time.
Eng/science is good if you just want to explore some pure science before you hit the engineering workforce, or give yourself a broader base for an engineering research career.
Civil/architecture sounds good, but it takes 5 years and then you have to do 2 more to qualify as an architect - which is really competitive anyway - so it's an onerous route to be an architect. If you want to do it and then be an engineer, you have to question the value of all those art studios towards your future work, but maybe you love art?
Civil and civil (structural) are the same degree, you just pick your specialisation in 3rd & 4th year anyway.

I have Ext 2 so can't really comment - it would be harder without, but if you are just looking at civil, you could do it with enough work.
