i think it's good to include date of birth, even for jobs that don't have a specific age for purposes such as wage determination. however, i wouldn't include birth certificate or anything unless they specifically request it.

Why's that?

It would bring discrimination against you getting an interview if you were 18 and other candidates are 16. I'd say lose the D.o.B

Unless there's a real need for D.o.B to be included, leave it.
