Hi there,
I'm looking for Korean people (any age, sex, etc) to fill in this questionnaire. I am doing my PIP on about attitudes of Korean people towards cosmetic (plastic) surgery and your response will be greatly appreciated. Your answers will remain confidential and only used in my PIP. Please answer all questions that you can answer, but if you feel uncomfortable with answering something, then feel free to leave it blank. Thank you so much!

PS SORRY THIS LOOKS LONG, BUT MOST QUESTIONS ONLY REQUIRE VERY SHORT ANSWERS! Only Korean people can answer, as they are the cultural group I'm focusing on.
1. What is your gender?
2. What is your age group? (13-16, 17-20, 21-29, 30-40, 41-59, or 60+)
3. Have you ever had cosmetic surgery? (if not, please go to Q10).
4. How many times have you had plastic surgery? Have you ever had plastic surgery more than once on the same body part? Why?
5. What kind of procedure/s have you had done? (eg. Rhinoplasty)
6. How much were you influenced by Korean celebrities to have your surgery?
7. Who primarily encouraged you to get plastic surgery? (Yourself, parents, friends, Korean celebrities/media, Western celebrities/media, Other - please list).
8. Why did you get plastic surgery?
9. Do you plan on having more cosmetic operations? Why?
10. Do you believe that people with plastic surgery are more attractive than people who haven’t had it done? (Yes/no/depends on the person)
11. Do you believe that people who have had surgery are more successful? In what ways? (eg. finding a partner, getting a better job, being considered more important)
12. In your opinion, is Korean plastic surgery on the rise? Can you suggest reasons for this?
13. Why do you believe Korean people get plastic surgery?
14. Roughly, what percentage of all the Korean people you know have had plastic surgery? Are most of these people teenagers, 20-30, 30-50 or over 50?
15. Do your parents have plastic surgery? What do they think of surgery and does this influence YOUR perspectives?
16. Would you get plastic surgery if it were without charge and risk-free? Yes/No/Maybe
17. Would Western celebrities (example, with blonde hair and big blue eyes) encourage you to have surgery? (Please rate 0-10, 0 being NO and 10 being YES)
18. Please rate your likeliness of getting surgery in the future (Not likely, very likely)
