This survey forms part of my HSC Community & Family Studies Independent Research Project, investigating the effects of family type on the wellbeing of children.

Please tick the appropriate box. Feel free to include comments at any stage.

* A nuclear family consists of a husband, wife and their biological/adopted children living in the same house. ***(For the purposes of this survey, ‘nuclear family’ will include de facto relationships.)
* A single parent family consists of one parent living with one or more of their children.
